Noida. The 30-year-old Geetika (name changed) was suffering from extremely severe jaundice (Level 23) due to the severely narrowing of the Common Bile Duct (CBD). Because of this, the way of going to the bile (digestive enzymes) till the intestine was blocked. But the doctor of the Reality Super Specialty Hospital, Greater Noida, made her successful after her successful operation.
The patient was brought to a critical condition in emergency and needed immediate medical attention. A detailed examination revealed that the upper part of his CBD (in which the gall bladder helps deliver bile to the small intestine) has become very narrow. Since the flow of bile was blocked, the level of bilirubin increased very much and due to this hyperbilarubinemia (jaundice) had reached very dangerous level.
The probable cause of jaundice at such a high level is believed to be either CBD injury or cancer. To identify the real cause of the disease, the team of doctors also investigated the old medical history of the patient. The patient had undergone cholecystectomy (removal of a gall bladder through surgery) last year, which may have been the major or responsible reason for his current condition. In this surgery there is a possibility of common bile duct wounds (unintestinal) and in a year it may be gradually narrowing. Because of this, his liver was severely swollen and the patient's condition deteriorated over time. Hence, he was advised to undergo immediate surgery.
A special team of doctors from the Department of Surgical Gastroenterology, under the leadership of Dr. Neeraj, conducted a detailed investigation of the patient. It was concluded that the patient urgently needed a hepaticojejunostomy, a surgical procedure of an artificial bypass to reach the small intestine straight from the narrow CBD.
Dr Neeraj, Senior Consultant and Head of Gastrointestinal, HPB, Minimally Invasive Surgery and Liver Transplant at Sarik Super Specialty Hospital, Greater Noida, said, 'The patient was prepared for the surgical procedure. Despite the high-risk procedure and the patient's worsening, the narrow section was carefully pulled out and artificial bypass was made to make a direct route to the bile's small intestine. The patient was kept under intensive medical supervision in the ICU for 5 days and when she recovered completely, she was discharged on the sixth day of surgery. After the operation, she started recovering fast and all the symptoms of jaundice were completely eradicated and bilirubin levels had also come down to normal. The patient was advised to remain under doctor's supervision for a month. '
Such cases are rarely seen in the youth and Geetika was also living a healthy lifestyle. After early symptoms were noticed in early March, Geetika made rounds of several major hospitals but yielded no favorable results. It took him six months to get proper diagnosis and treatment due to a lockdown between Kovid.
